Responsibilities
Conduct comprehensive assessments to understand patients’ medical, social, and emotional needs.
Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers.
Assist patients in accessing resources such as medication, financial assistance, and community support programs.
Monitor and document patients’ progress, adjusting care plans as necessary to meet changing needs.
Advocate for patients by coordinating with insurance companies, healthcare providers, and other organizations.
Provide education and emotional support to patients and their families to promote health and well-being.
Qualifications
-Education: Bachelor’s degree in social work, psychology, healthcare, or a related field (required); master’s degree preferred.
-1-3 years of experience in case management, social work, or a related field (healthcare setting preferred).
-Strong interpersonal and communication skills.
-Ability to manage multiple cases efficiently and prioritize tasks effectively.
-Problem-solving mindset with a focus on patient-centered care.
-Proficiency in documentation and data management software.
-Certifications: Certification in case management (CCM) or equivalent is a plus.
-Language: Bilingual capabilities are a significant asset.
